Residential Services


QMRP (Qualified Mental Retardation Professional)
Responsible for supporting people in the attainment of their personal outcomes and desires, responsible to assure that each person served is choosing and achieving their Personal Outcomes in their home and community. QMRP demonstrates knowledge of Personal Outcome Measures/CQL and Title 19 regulations. QMRP acts as a liaison between the person, his/her family and/or guardian, and other support networks. They are responsible to coordinate annual Life Plan and other meetings as necessary during the Plan Year. They are responsible to identify required and additional skill assessments for Life Plan development. They are responsible to distribute skill assessments annually and update through the Plan year. They are responsible to assist and support people as they identify and work toward their personal goals and outcomes. They are responsible to solicit and respond to person served feedback as to their wants and needs as they change. To develop Life Plans based on established personal outcomes and objectives. They are responsible to ensure written instructions or methods are readily understood and clear. They are responsible to provide in-service training after each Life Plan and its changes during the Plan year. Behavior Intervention Plans will be in-serviced following BIRC direction and with each new Support Staff. They are responsible to monitor plans to ensure that the supports are in place and effective. Quarterly reviews will be conducted to measure and/or assess Outcomes/Processes are being met, this will be documented in the case file, distributed to other team members and entered into a computer database fro tracking monthly. Respond timely to persons served for desired changes expressed about their Plan and if progress is not evident upon monitoring. There are on average about 20 people served on an individual QMRPs' caseload. Each QMRP will be on site minimally once to twice a week in group settings, twice a month in 24 hours independent living settings and at least monthly on non 24 hour independent living settings. The observations and/or recommendations from site visits are documented on the QMRP notes and monitoring tool. Monitoring of day support and/or education programs is also expected on a regular basis. Support to SLA customers is at least quarterly and as needed. Will work to establish a positive and respectful relationship with each person served and assist and/or support them in their day to day routines within their home and community. Personal care support is provided as needed. Will support people in to participate in community activities and enhance or build relationships. They are responsible to ensure health and safety needs are addressed. They assure that a comfortable, safe, and sanitary living environment is maintained. Attends staff meetings and provides individualized and group instruction. Writes and speaks clearly and informatively. They ensure that all staff training is conducted and documented. They are someone with a positive attitude and are self-confident, caring, responsible, and dependable. Balances team and individual responsibilities while ensuring quality and timeliness of tasks. Responsibly handles all personal monies and management of benefits. Require the ability to be responsive to the demands or directions of multiple people or interests. Other duties assigned include rotating weekend/holiday and storm team on-call procedures.

Residential Manager
Residential Manager?s are responsible for supporting people in the attainment of their personal outcome and desires. They will work to establish a positive and respectful relationship with each person served and assist and/or support them in their day-to-day routines within their home and community. Personal care support is provided as needed. To support and assist all Residential Support Staff (from 5 to 25 staff) with daily scheduling to ensure that personal outcomes are met. They are responsible for the continuous direction of the daily operation of the group home or Independent living caseload (from 6 to 10 people). They are responsible for ensuring proper staff ratios through interviewing and hiring, supervision, performance evaluation and recognition of Residential Support Staff. They are responsible for participation and completion of assessments, service planning and implementation of Life Plans. Will support people to participate in community activities and enhance or build relationships. They are responsible to ensure health and safety needs are addressed. They assure that a comfortable, safe and sanitary living environment is maintained. Prepares for and facilitates staff meetings. Writes and speaks clearly and informatively. They ensure that all staff training is conducted and documented timely. They are someone with a positive attitude and is self-confident, caring, responsible and dependable. Inspires and motivates Direct Support Staff (DSP & DPL) to perform their duties and responsibilities through positive coaching and leadership daily. Balances team and individual responsibilities while ensuring quality and timeliness of tasks. Works to remain within the approved fiscal operational budget and/or hours for the corporation as well as for the person served. Responsibly handles all personal monies and management of benefits. They require the ability to be responsive to the demands and directions of multiple people and interests. Other duties assigned include on call responsibilities via cell phone and/or pager Monday through Friday 24 hours a day.

Direct Professional Leader
Position is responsible for supporting people in the attainment of their personal outcomes and desires. Will work to establish a positive and respectful relationship with each person served and assist and/or support team in their day-to-day routines within their home and community. Personal care support is provided as needed. Will support people to participate in community activities and enhance or build relationships. Responsible to ensure health and safety needs are addressed. The DPL helps to keep a neat and clean living environment. Writes and speaks clearly and informatively. Completes the on going training that is provided. Is someone with a positive attitude and is self-confident, caring, responsible and dependable. Inspires and motivates Direct Support Professional to perform their duties and responsibilities through positive coaching and leadership daily. Balances team and individual responsibilities while ensuring quality and timeliness of tasks. Works within approved budgets for both the organization and persons served. The DPL displays willingness to make decisions while exhibiting sound judgment. Other duties assigned may include Food Services, Community Services and support for Residential Manager in their absence and on call responsibilities via cell phone and/or pager.

Direct Support Professional
They are responsible for supporting people in the attainment of their personal outcomes and desires. They will work to establish a positive and respectful relationship with each person served and assist and/or support them in their day-to-day routines within their home and community. Personal care support is provided as needed. They will support people to participate in community activities and enhance or build relationships. They are responsible to ensure health and safety needs are addressed. They help to keep a neat and clean living environment. Writes and speaks clearly and informatively. They will complete on going training that is provided. Is someone with a positive attitude and is self-confident, interactive, caring, a good listener, responsible and dependable.

Adult Services Coordinator
This position is responsible for the direction, coordination, and evaluation of assigned program units including but not limited to the performance of people we support and the success of program delivery, fiscal accountability of programs, and the development of new residential and community supports. This position will serve as an active member of a multi-discipline administrative team for the Residential Department and would provide Department coverage in the absence of the Residential Services Director. The Adult Service Coordinator supervises multiple Residential Managers who supervise Residential Support Leaders and Staff within their individual program units. This position also supervises Qualified Mental Retardation Professionals. The Adult Service Coordinators share in the back up responsibility of the Department Emergency Relief system. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; recognition and addressing complaints and resolving problems. Ensures services are delivered in a safe, healthy environment that is free from abuse and harm and in compliance with standards of excellence as set forth by the Council on Quality Leadership or CQL and Title 19 regulations. Will ensure the development and support for community life is present so that people can participate in community activities and enhance or build relationships. Demonstrates knowledge of benefit management. Requires the ability to be responsive to the demands or directions of multiple people or interests. Other duties assigned include rotating weekend/holiday on-call, storm team & cell responsibility for emergency relief system M-F. Flexibility is essential in this position.

Licensed Practical Nurse
The Licensed Practical Nurse is expected to facilitate the attainment of Personal Outcomes by upholding the goals and philosophy of Development Homes, Inc., abide by the policies as stated in the organizational/personnel policy, Board of Nursing Regulations, and by performing the responsibilities outlined as follows: Treat persons served with dignity and respect, listen to their desires, concerns, and opinions, and give them choices. Perform and respond to physical assessment functions for persons served to ensure they have the best possible health. Implement and monitor the medication administration process to ensure physician?s orders and medical professionals recommendations are followed. Provide support and monitoring for persons served to obtain health care and be involved with their own medical care to their developmental level. Provide support and training to employees throughout the organization to ensure persons receiving services are safe, medical concerns are addressed, and they experience service continuity. Assists with maintaining medical equipment to ensure necessary assessments and intervention can be completed and to enable people receiving services to use their environment. Participate in the Interdisciplinary Team process to assist people to choose their personal goals and services, achieve their personal goals, and have the personal outcomes they want in their lives, including best possible health. Follow risk management program and behavior intervention policies to ensure self, co-workers, and persons served are safe. Persons served are free from abuse, neglect, and exploitation. Manage the medical records. Communicate verbally and in writing relevant information regarding persons served based on what they chose to share following confidentiality and HIPAA policies. Display professionalism and work as part of a Team to provide quality health care. Complete training requirements. Perform other duties and responsibilities as assigned by the supervisor. Work a rotating weekend on-call for the company and be responsive to calls for support during the week.
